Domains developed

What you'll practise

TRANSITION develops 5 domains (areas of everyday life) picked for this stage. Your Outcome Report scores all 5 at the start and end. Your goal sits inside one of them.

  • 1Main focus

    Choosing what's next

Exploring courses, work and day options, then taking the first real step.

    Life long learning
  • 2Main focus

    Getting places, handling money

Planning a trip, catching the bus and paying your own way.

    Community, social and civic life
  • 3

    A routine of your own

Getting up and getting going when the school bell no longer does it for you.

    General tasks and demands
  • 4

    New places, new people

Starting conversations where nobody knows you yet.

    Interpersonal interactions and relationships
  • 5

    Bigger decisions

Weighing up options and managing your own time.

    Learning and applying knowledge

Taught your way. Your diagnosis shapes how we teach, and every activity comes in three levels, from more support to more challenge.

Practised where it counts. At home and out in your community, where you're most comfortable and where the goal matters. Not in a clinic.

Built to change how much help you need, not just to tick off a goal. How we measure it →

Outcome Report

Every change documented. Confirmed.

Therapy shouldn't end with a guess. Your Outcome Report scores your 5 domains at the start and the end, in the functional capacity language the NDIS uses, so everyone can see what changed.

  • See the change. Progress you can see, not just feel.
  • Get your support team on the same page. Family and supports keep building on it. You choose who sees it.
  • Know what's next. What to keep practising.

Questions about TRANSITION

Is TRANSITION the same as SLES?

No. School Leaver Employment Supports are a separate NDIS employment support. TRANSITION builds everyday life skills and can run alongside SLES or Impact6 WORK.

SupportWhat it's forRuns alongside
TRANSITIONEveryday life skills for leaving school: routines, travel, money and what comes nextSLES or WORK
SLESA separate NDIS employment support for school leavers, not run by Impact6TRANSITION or WORK
WORK 16+Practice for finding and keeping a job, paid from Finding and Keeping a JobTRANSITION or SLES
Can I do TRANSITION and WORK together?

Yes. WORK is for ages 16 and over and is paid from a different budget, Finding and Keeping a Job, so the two can run side by side.

When should we start?

Term 3 or 4 of the final school year works well. That way the skills are in place before school ends.

Can family or support workers be there?

Yes, and for anyone under 18 one adult has to be. A parent, guardian or adult carer you name in writing is present for the whole of every session: in the home within sight or hearing, and along on every outing. If no adult is there, the session can't go ahead and is charged as a short-notice cancellation. During activities the adult watches without stepping in. Other family and support workers are welcome to join any session too.

What if they won't join in?

Every session follows the same predictable shape, the work starts at a level they can manage, and they can say no to any activity. If it still isn't working, stop any time and pay only for the sessions delivered.

What if it isn't working?

Stop any time. You pay only for the sessions you've had, billed after each one. There's no exit fee.

Who comes to our home?

A registered allied health practitioner, such as an occupational therapist, developmental educator, speech pathologist, social worker or psychologist. They're registered with AHPRA or accredited by their professional body. Every practitioner holds a current NDIS Worker Screening clearance and, for participants under 18, a Working with Children Check where state law requires one. We keep the same practitioner throughout wherever possible.

Why at home and not in a clinic?

That's deliberate. People learn best where they feel comfortable, and home and the local community are where the goal actually matters. A skill practised in your own kitchen or on your own bus route is one you can use the next day, not one you have to work out again somewhere else.

Does this replace our current therapist?

No. They do different jobs. Ongoing therapy supports you over the long run. Impact6 is a short, focused block: twice a week for 6 weeks, the same practitioner, one goal practised in the places it matters, and a score at the start and the end. You can keep your regular supports going and add Impact6 as a concentrated push, if your plan has room for both. With your consent, we'll share the Outcome Report with your therapist so everyone builds on the same progress.

Can I talk to someone first?

Yes. Book a call time that suits you and we'll ring you then.
