We collect personal information necessary to deliver high-quality, safe disability supports and services. This includes:

  • Identity Data: Your full name, date of birth, gender, and NDIS participant number.
  • Contact Data: Your phone number, email address, physical address, and emergency contact details.
  • Sensitive Data: Detailed health records, details of your disability, medical histories, and NDIS funding plans.
  • Financial Data: Your bank details or plan management details for invoicing and processing payments.
  • Website Data: Technical data like your IP address, browser type, and page interactions collected via cookies when you browse our website.

3. How We Collect Your Information

We collect information directly from you when you fill out website contact forms, complete service agreements, or speak with our staff. We may also collect information from third parties (such as your NDIS Plan Manager, Support Coordinator, or local area coordinator) only after obtaining your clear, written consent.

4. How We Use Your Information

We use your information strictly to:

  • Verify your identity and evaluate your NDIS service eligibility.
  • Create, manage, and safely deliver your individualised NDIS support plans.
  • Coordinate care with your health professionals and authorised representatives.
  • Process government claims, service invoices, and portal payments.
  • Meet our compliance reporting obligations to the NDIS Quality and Safeguards Commission.

5. Disclosure of Your Information

We do not sell, trade, or rent your personal information to anyone. We only disclose your data to third parties:

  • With your explicit, informed written consent.
  • To contracted health or allied professionals directly assisting with your care.
  • When legally required to report child safety concerns, severe abuse, or imminent safety threats to the police or the NDIS Commission.

6. Data Security and Retention

We store your records securely using encrypted cloud servers and secure physical filing. Only authorised staff members have restricted access to your files on a need-to-know basis. We retain records for the legal periods mandated under Australian disability laws and securely destroy or permanently de-identify files once they are no longer required.

7. Your Rights: Access and Correction

You have the right to request access to any personal information we hold about you. You can also request an immediate correction if you believe our data is inaccurate, outdated, or incomplete. To make an access request, please contact our Privacy Officer using the details below.

8. Privacy Complaints

If you believe we have breached your privacy, you can submit a formal complaint directly to us. We will investigate and respond to your complaint within 14 days.

If you are unsatisfied with our response, you can escalate your complaint externally to the Office of the Australian Information Commissioner (OAIC) or contact the NDIS Quality and Safeguards Commission directly.
