Who we are
Impact6 is a program of Capacity Builders Skilling & Training Pty Ltd (ABN 21 639 524 495), a registered NDIS provider (registration 4053448890), at Suite 284, Tenancy 111, Camberwell Place, 793 Burke Road, Camberwell VIC 3124. In this policy, “we” means Capacity Builders.
We follow the Privacy Act 1988 (Cth) and the Australian Privacy Principles. Because we deliver a health-related service, we treat health information with extra care.
What we collect
On this website
When you use the plan check, book a start week, book a call, refer a participant or send a message, we collect what you type in: names, email, phone, suburb and postcode, the participant's age, the program, the plan type and budget, and your answers to the plan check. We don't ask for health details on the website, and we ask you not to include them.
When a program starts
Your service agreement collects the participant's full name, date of birth, address, NDIS number, plan dates and plan manager details, and who we may share progress with. During the program your practitioner records session notes, the goal you set, and the scores on the support scale at the start and the end. Sessions may be recorded for quality checks, with your consent. If you'd rather not, a session can be watched live by video instead. You can withdraw your consent at any time, and it won't affect your program. Some of this is health information, which is sensitive information under the Privacy Act. We collect it only with consent and only because we need it to deliver the program.
How we collect it
Mostly from you, or from someone acting for you, such as a parent, guardian, nominee or support coordinator who has your agreement. If we receive information we didn't ask for and don't need, we destroy or de-identify it.
Why we use it
- To arrange the program
- To deliver sessions and write the Outcome Report
- To claim payment from the NDIS plan or plan manager
- To meet our duties as a registered NDIS provider, including incident reporting and complaints handling
- To improve Impact6 using de-identified information, only if you give separate consent. Saying no doesn't change your program
We won't use identifiable information for research without your separate written consent. We never use photos or videos of you in our marketing without your separate written consent.
Who we share it with
- Your practitioner
- Your plan manager or the NDIA, to claim payment
- People you tell us to share progress with, such as your support coordinator, guardian or therapist
- The NDIS Quality and Safeguards Commission, or others, where the law requires it
- Companies that run systems for us, under contract and only to provide that service
Systems we use, and where information is stored
- Website and website forms: Netlify, a US company. Website form entries may be stored outside Australia. That's one reason we don't ask for health details on the website.
- Moving website form entries into our client records: n8n, an automation service hosted in the European Union. It passes the details you type on a website form to Splose and keeps only record numbers, dates and yes/no flags, never names or contact details.
- Client records and scheduling: Splose.
- Outcome Reports: made in Splose, from your client record.
Where information may be stored overseas, we take reasonable steps so it's handled in line with the Australian Privacy Principles.
Cookies and analytics
This website doesn't use advertising or tracking cookies.
Keeping it safe
We limit access to people who need it, use systems with secure sign-in, and keep records for as long as NDIS and other laws require. After that, we securely destroy or de-identify them. If a data breach is likely to cause serious harm, we'll tell you and the Office of the Australian Information Commissioner, as the law requires.
Seeing or correcting your information
You can ask for a copy of the information we hold about you, or ask us to correct it, at any time. Email support@capacitybuilders.com.au or call 1300 894 991. We'll respond within 30 days. If we can't give you access, we'll explain why in writing.
Complaints about privacy
Tell us first, through Feedback and complaints or at support@capacitybuilders.com.au. We'll acknowledge it within 2 working days. If you're not satisfied, you can contact the Office of the Australian Information Commissioner at oaic.gov.au or on 1300 363 992.
