What you'll practise
SUSTAIN develops 5 domains (areas of everyday life) picked for this stage. Your Outcome Report scores all 5 at the start and end. Your goal sits inside one of them.
- 1Main focus
Looking after yourself, a new way
Same tasks, a method that fits the body and energy you have now.
Self care - 2Main focus
Home tasks, re-set
A new approach, piece of equipment or set-up for familiar jobs.
Domestic life - 3
Rebuilding the routine
A week that works around what has changed.
General tasks and demands - 4
Staying connected
Keeping the places and people that matter within reach.
Community, social and civic life - 5
Health and service conversations
Getting what you need from appointments and services.
Communication
Taught your way. Your diagnosis shapes how we teach, and every activity comes in three levels, from more support to more challenge.
Practised where it counts. At home and out in your community, where you're most comfortable and where the goal matters. Not in a clinic.
Built to change how much help you need, not just to tick off a goal. How we measure it →

I can get ready with my set-up.I can do the washing again.I can plan a week that works.I can still get to my club.I can speak up with my GP.Can is the moment a task becomes yours. Not done for you. Done by you, because you practised it where it happens. Every can is a step toward the independence you choose.
Is this for you?
- You're 45 to 64 with Improved Daily Living funding
- Something has changed, like your health, your energy or the person who always helped, and the old way doesn't fit any more
- You want to keep doing it yourself, a different way
Not sure? The plan check answers it in 2 minutes. Age is only a guide, so choose the program that fits what you're facing now.
Every change documented. Confirmed.
Therapy shouldn't end with a guess. Your Outcome Report scores your 5 domains at the start and the end, in the functional capacity language the NDIS uses, so everyone can see what changed.
- See the change. Progress you can see, not just feel.
- Get your support team on the same page. Family and supports keep building on it. You choose who sees it.
- Know what's next. What to keep practising.

Questions about SUSTAIN
Is it too late to learn new skills?
No. People keep learning across the whole of life. SUSTAIN doesn't start from scratch. It adapts how you do things you already know.
The person who always helped me can't any more. Is this for me?
Yes. When the person who always helped changes, the old way can stop working. SUSTAIN helps you find a new way to do those tasks yourself, at home and in your local area.
Will I have to go anywhere?
Sessions happen where the task happens, usually at home and in your local area.
Which budget pays?
Capacity Building: Improved Daily Living.
Can family or support workers be there?
Yes. Parents, guardians and support workers are welcome at any session, or every session, with the participant's OK. They see what's being practised and learn how to back it up between visits. For a participant under 18, a parent, guardian or adult carer must be present for the whole of every session, including outings.
What if it isn't working?
Stop any time. You pay only for the sessions you've had, billed after each one. There's no exit fee.
Who comes to my home?
A registered allied health practitioner, such as an occupational therapist, developmental educator, speech pathologist, social worker or psychologist. They're registered with AHPRA or accredited by their professional body. Every practitioner holds a current NDIS Worker Screening clearance and, for participants under 18, a Working with Children Check where state law requires one. We keep the same practitioner throughout wherever possible.
Why at home and not in a clinic?
That's deliberate. People learn best where they feel comfortable, and home and the local community are where the goal actually matters. A skill practised in your own kitchen or on your own bus route is one you can use the next day, not one you have to work out again somewhere else.
Does this replace our current therapist?
No. They do different jobs. Ongoing therapy supports you over the long run. Impact6 is a short, focused block: twice a week for 6 weeks, the same practitioner, one goal practised in the places it matters, and a score at the start and the end. You can keep your regular supports going and add Impact6 as a concentrated push, if your plan has room for both. With your consent, we'll share the Outcome Report with your therapist so everyone builds on the same progress.
Can I talk to someone first?
Yes. Book a call time that suits you and we'll ring you then.
