From your first session to your report.
- Before
Check, choose, sign
Do the plan check, pick a start week, and sign your service agreement online. Your end date is set before your first session.
- Week 1
Where you're starting
Your practitioner sees how you manage the 5 domains your program builds, and you choose your goal. Practice starts the same day.
- Weeks 1 to 6
Practice, twice a week
Two 2-hour sessions a week, in the places your goal happens. Each week builds on the last.
- Between sessions
Keep practising
A small task to try before the next visit. Family or support people learn their part.
- Week 6
Where you finished
The same practitioner looks at the same 5 domains again, and rates your goal against the levels you set in week 1. You add your own words.
- Within 5 working days
Your Outcome Report

- After
A quick check
We send a short feedback form 4 and 8 weeks later. If more support would help, we'll tell you what and why.
Every 2 hours follows the same shape.
So you always know what's coming. Week 1 and week 6 sessions also include the start and end scoring.
Practised where it counts. At home and out in your community, where you're most comfortable and where the goal matters. Not in a clinic.
Why at home and not in a clinic?
That's deliberate. People learn best where they feel comfortable, and home and the local community are where the goal actually matters. A skill practised in your own kitchen or on your own bus route is one you can use the next day, not one you have to work out again somewhere else.
Built to change how much help you need.
That's functional capacity: how much support you need to do the everyday things in your life. Impact6 builds it across the 5 domains your program covers.
Your practitioner scores each domain on this scale in week 1 and again in week 6. Your goal sits inside one of those domains and gets a reading of Achieved, Progressed or Not yet.
A goal is never refused. If it sits outside the program's 5 domains, we work on the closest part that fits and record your goal in your words.
Pervasive
Two or more people needed at once
Extensive
Someone does most or all of it for you
Moderate
Someone helps you through the steps
Minor
A light touch: one prompt to start, or a quick check
Managed independence
You do it alone, with your own app, tool or system
Independent
You do it without help
Example of a move: from someone helping through the steps (3) to one prompt to start (2). 6 weeks will sometimes move a domain and sometimes not, and your report says which.

The areas of life Impact6 builds.
The NDIS looks at functional capacity across domains: areas of everyday life, like running a home or getting along with people. Impact6 builds every domain that practice can change, and each program develops the 5 that matter most at that stage of life.

Every change documented. Confirmed.
5 domains, scored at the start and the end on the 0 to 5 scale above. A lower score means less help is needed. Your goal is reported in words: Achieved, Progressed or Not yet. Delivered within 5 working days of your last session.
The full sample report shows a whole report: every domain, including any that didn't move, the goal, what the participant keeps and the recommended next step.
